In order to assist with the management of your medical request, it may be necessary for the practice to send a form for you to complete, the results of which will be returned to the practice for them to review.
If the practice have sent you a form for completion, you will receive a message similar to the below via askmyGP:
You can click the 'Fill in form' button to proceed and from here you will see the various questions that will need to be completed:
Most questions will simply be 'Yes' or 'No' questions, or multiple choice, but other question formats may be presented. You will need to answer all questions on the form (if you try to proceed without answering a mandatory question, the system will let you know).
Once this is done you can click the green 'Complete' button to send your response back to the practice.
Note: If you are midway through answering the questions and need to stop temporarily, you can choose 'Save progress' to record your response so far, allowing you to return to the form at a later date to complete the rest of the questions from where you last left them.
