Forms (Practice)

Our clinical forms tool allows you to send patients pre-defined forms for their completion. Forms can also be completed on behalf of the patient via telephone, meaning choice of channel doesn’t affect quality of service.

Forms can also be used to configure custom patient pathways, for example you can configure a button in the patient portal which they can click to immediately raise a request using a Sicknote form (See here for more details)

Sending a Form to the patient
To send a form to the patient, click the 'Forms' button on the request screen:

This will open the 'Choose a Form' window, where you will be able to select the form you wish to send to the patient (or search for a specific form):

Clicking the form you require will attach it with a pre-populated message, which you can amend, or delete should you wish to. Your message and the form can then be sent to the patient by clicking 'Send':

Completing a Form on the Patient's behalf
Once the form has been sent to the patient, you may wish to complete it on their behalf, if for instance you wished to go through the form with them if they were on the phone to you at the time.

In the request thread, there will be a button you can select which will open the form for you to complete 'Fill in form (form name)':

After clicking the button, you can work your way through completing the form, once you have answered all of the questions you can click 'Complete' to save the responses and close the form, alternatively you can click 'Save Progress' if you need to save the progress so far and resume later, or you can click 'Cancel' to back out of the form.

Note: You can also add the form as an 'Internal note', rather than a 'message to patient' to complete it, without the patient having visibility of the form, or it's responses.


Patient Completed Form
In most instances, you will require the patient to complete the form themselves. After sending the form to the patient, they will be notified of a new message in their app and their request thread will have updated, allowing them to click through to complete the form:

Form Completed
Once the form has been completed by either the practice, or the patient, the request thread will update for the practice to provide a summary of the responses, any scores and totals and will also include details relating to who completed the form (patient or practice) and the date/time that the form was completed:

If you wish to review the completed form in more detail, you can select 'View form (form name)', you will not be able to make any changes to the completed form, but you can view the responses and any scores/totals.

Copying the Form Data
Once the form has been submitted and the request has been completed, the copy box will appear on screen as usual, this copy box will now also include the information relating to any completed forms within that request. You will be able to see the questions, replies, scores and totals. You can hit the 'Copy' box to copy all of the information to your clipboard and then paste into the clinical system.

Coding
Note, some forms will automatically add a code to the patient request, you can remove these at the point of completing the request should you wish, or include them to be written back to the patient record. For more information, please see click here
