If you have spent a few minutes to triage online and then decide to bring the patient in, do you reduce the appointment time?

Usually the nominal time is the same due to the information you have already gathered, but it can be used more productively.  Remember that in a demand-flow system, slot length is irrelevant, you give each patient what they need, no more and no less.
