Creating a patient record

If your patient doesn't have access to the internet and needs you to create their record for them, this guide shows you how.

Select the 'Patients' tab on the left-hand side.

Select the plus button at the bottom of the screen.

NHS Spine Lookup: Click on ‘NHS Spine lookup’ to populate the User Details with information currently held by this patient.

Enter Patient details: DOB, Surname and Gender fields are a minimum. Then click on 'Search.'

Once a match has been returned, you will be able to view the Spine details and the askmyGP details for the patient in a side-by-side table:

You will see checkboxes next to some fields in the 'copy' column, and you can choose whether you wish to retain the details held in askmyGP by leaving the box unticked, or if you wish to 'copy' the information held in the Spine over to the patient's askmyGP record and replace what is currently stored, this is done by ticking the 'copy' checkbox.

Note: It's important to note that if the email address held in the spine differs from the one in the askmyGP record, choosing to 'copy' this over will replace the existing email address and prevent the patient from being able to access their account online, so we would not recommend updating the email address, without first consulting the patient.

Other details:

If the patient has a usual GP, find them under the ‘Usual GP’ drop-down.

If you are a multi-site practice, select the preferred site for the patient.

Patient verification status:

Because you have used the NHS Spine lookup you can change the ID status to 'Identified' by clicking on the drop-down arrow.

Once you have seen or spoken to the patient and you are sure of who they are, change the ID Status to 'Verified'. (Note: If the patient makes any changes to their registered details they will become 'unverified' and their ID status will move back to 'Claimed')

Proxy requests:

If this patient is going to be making requests on behalf of other patients eg their children, then link their records together by clicking on 'Add' and finding the other patient. NB You will need to have created the other patient's account first before you can link them.

Conversely, if someone else will be making requests on this patient's behalf, then click on 'Add' and link the patients together. NB You will need to have created the other patient's account first before you can link them.
